Contact OMMA:

Patients and Caregivers:

  • Card Never Received: If you have never received your card, please DO NOT fill out a replacement request in the OMMA application portal. Please check with your post office and fill out the Patient Contact Form.
  • Replacement Cards: Patients and caregivers who have received their card but need a replacement can now request replacement cards through the OMMA application portal. To request a replacement card, login to your account, select "create new application" and select "patient card replacement" or "caregiver card replacement." You will need to submit the replacement card application, and OMMA will respond by email.
  • Updates/Changes: Patients and caregivers can now make updates to their information through the OMMA application portal. To update your information, login to your account, select "create new application" and select "patient update" or "caregiver update." You will need to submit the update application, and OMMA will respond by email.  
    • Note: Date of birth and email address cannot be updated through the application portal. To request those changes, please fill out the Patient Contact Form.
  • Business Complaints: Complete and submit the OMMA Business Complaint Form if you wish to file a complaint on a licensed medical marijuana business. Please provide as much detail as possible.
  • Other Items: For all other questions or requests, please fill out the Patient Contact Form.

Businesses:

  • Updates/Changes: Business licensees who wish to update contact information, location, and/or ownership information may now do so through the OMMA application portal. Submit a change request by logging into the OMMA application portal and selecting “Create New Application” and then “Business License Update.” You will then select the “Create Application” button to fill out and submit the update request.
  • Compliance Questions: Please visit http://omma.ok.gov/compliance-information or email OMMACompliance@ok.gov.
  • Business Complaints: Complete and submit the OMMA Business Complaint Form if you wish to file a complaint on a licensed medical marijuana business. Please provide as much detail as possible.
  • Other Items: Please email OMMAGrower@ok.gov, OMMAProcessor@ok.gov, or OMMADispensary@ok.gov.

Physicians:

  • Please email OMMAPhysicians@ok.gov for questions about completing the OMMA physician registry or eligibility guidelines for recommending medical marijuana to patients.

Please Note: All application status updates are automatically sent to the email address provided in the application, including rejections. If your application is rejected after OMMA reviews your application, you will be able to make changes to your application once you receive the rejection email.
